　　首先，假條的上方還是應該按照半正式的格式寫上如下信息： to：假條是遞給誰的 from：請假人

　　date：寫假條的日期(注意不是請假的日期) subject：寫上請假字樣(事假或病假)

　　其次，在您的請假信第一段，應該開門見山但是有禮貌地提出請假。第一段要中心明確，寫清您要請假的日期。
